NOTES FROM THE NURSE

Hello Parents of Farnsworth/Cleveland Students! My name is Terri Smith.  I would just like to express how much I am enjoying getting to know your child and working with them as your new School Nurse this year.  I will be at both sites.  I hope we can work together to promote and maintain the health of our students at its best level. We recently screened all 7^th graders for vision.  If your child failed this screening you will be receiving a referral soon.  Please have your child checked by an eye doctor as soon as possible.  If you need assistance with this, please give me a call.

Early Childhood Screening.  If you have a young child age 3-5 years old please contact ECS at 651-632-3746 to set up an appointment to have your child screened.  This is required by law and allows our school district to provide early education opportunities for your child if he/she qualify.

SHOTS!!!  There are still many 7^th graders who have not received the required immunizations to attend 7^th grade! 2 letters have been sent home and action is now being taken to exclude students from school. This action not only hurts our students with unexcused absenteeism and lost learning time, it hurts our whole school. If you have received a letter stating your child needs shots for 7^th grade, please make a clinic appointment today. Please contact me at 651-293-8880 ext.1303 if you have questions. Send the dates the shots were completed to me as soon as possible after the appointment.
